Why this consent record exists.

Before opening any lock, a responsible locksmith confirms that the person requesting entry has the lawful right to access the property or vehicle. This form records that consent and your proof of ownership.

What you’ll need:
  • Your name & NRIC / passport number
  • The locksmith’s GPPKM member ID — verify it on the registry
  • A photo of the property / vehicle (max 5 MB)
  • A photo of your IC / passport front (max 5 MB)
Declaration. By submitting, you declare you have the lawful right to access the stated property or vehicle. GPPKM is the trade association and does not dispatch the job, set prices, or arbitrate the engagement — this record supports the locksmith’s due diligence.
02Consent & disclaimer form

Complete & submit.

1. I assure that the Company is acting lawfully by helping me to access the premises stating categorically, and my entry into premises is in no way deceitful or fraudulent and does not infringe upon the rights of any others of the property's security.
2. I accept full and total responsibility for any liability and damages arising as a result of my request for the open lock service and make no claim against the company and leave them free of and indemity then against any future claims without prejudice or reservation.
3. I am hereby agreeing to the above said Term and Conditions with my authorize at my own free will and hereby attached with a copy of identity card.
